Welcome to J-ADD's website! J-ADD prides itself in its person-centered planning philosophy that uses life-planning tools like PATH and MAPS. In order to assist those individuals we support in the community make meaningful choices about where to live and what types of support and the manner of support they need, a plan of care is developed.
Key to the philosophy of person-centered thinking is learning to listen to what the individual has to say. J-ADD staff try to follow Michael Smull's five basic skills that serve as a foundation for person-centered planning. They are as follows:
1) Identifying how a person wants to live (what is important to the person) and balancing that with any health and safety issues (what is important for the person);
2)Define the roles and responsibilities of the people who are paid to support;
3)Finding a match between those who are paid and those who use the services;
4)Learning to work with the different ways that people communicate (especially people who are non-verbal);
5)Sort out that which works for the person and that which doesn't from both the person's perspective and from those around the person.

J-ADD uses a team model approach in conjunction with DDD's case management system in order to broaden the creative thinking that is generated. As a team of equal partners working together for the same goal, we are able to generate more comprehensive, successful outcomes for the individuals in our community.
